What does double chin removal in Korea actually involve?

Most people arrive expecting either a needle course or an operation. The route used here for a submental fat pad is V-Tight, and it sits between the two. A thin cannula is placed through one numbed entry beneath the chin, ultrasound energy liquefies the fat held in that pocket, and the liquefied fat is drawn back out through the same instrument during the visit. You are awake under local anaesthesia throughout, able to answer questions while the director works. The active part runs about twenty minutes and the appointment closes inside an hour, covering the area in a single attendance. That timing is the working difference from an injectable course, which normally means at least five weekly attendances before any change reads on camera. Whether a chin belongs in this category at all is decided face to face, since slack skin behaves nothing like a fat pad.

    Reading what is under your jaw

    A director puts a hand under the chin and works out what is filling the space: a genuine fat pad, skin that has lost its recoil, a heavy platysma band, or simply a low-set jaw angle. Only the first of those responds to extraction. You are told which one you have, in plain words, before anything else is discussed.

    Numbing a single point beneath the chin

    Local anaesthetic is placed through one small opening in the shadow under the jaw, where the skin creases naturally. You stay awake, sitting up between checks, and you can speak throughout. Sedation exists as an option for people who want it and carries an additional fee; the agent involved is discussed only in the room, never in published material.

    Ultrasound melt, then extraction

    The cannula passes into the submental pocket and ultrasound energy breaks the fat down into a liquid state. The same cannula then aspirates it out. This is the part patients ask about most and it is also the shortest: about twenty minutes of working time for the area, with the director moving in short passes and stopping to feel the contour by hand.

    Support garment, then you walk out

    A chin support is fitted before you leave and you are shown how to wear it at home. Aftercare instructions cover sleeping position, chewing, and what swelling looks like over the first week. You go straight back into your day; nobody drives you home, because nothing was given that would stop you walking out on your own.

The Method

Why V-Tight is described as melt-then-remove.

The submental compartment is small, curved, and sits directly over structures nobody wants disturbed, so the instrument that works well across a thigh is the wrong tool for it. V-Tight was built around that constraint. Ultrasound is applied first so the fat gives way easily and the cannula does not have to tear through firm tissue to travel; the softened fat is then aspirated through the same channel. Two actions, one entry point, one sitting.

That sequence is the reason the chin route is named separately from the body work the clinic is better known for. Under the chin, volume is measured in small amounts and the surface reads every irregularity from across a table, so softening the fat before removing it earns its place in the protocol.

It is worth being precise about the vocabulary, because search results mix it badly. An injectable lipolysis course puts a solution into the fat so the cells shrink and the body clears them through the urinary route over weeks. V-Tight removes the fat physically during the appointment. Both are marketed in English under overlapping phrases, and the words on a brochure will not tell you which one you are being sold. Ask what leaves the room with the clinician.

Timing

Twenty minutes of work inside a one-hour appointment.

International patients plan around flights, so the clock matters more here than it does for a local booking. The consultation, the marking, the numbing, the procedure itself and the fitting of the support garment are all completed within an hour of arrival. The ultrasound and extraction portion accounts for around twenty minutes of that. There is no separate surgical admission, no fasting instruction the night before, and no requirement to arrange a companion to collect you.

One attendance covers the submental area. That single-visit structure is what people usually mean when they compare this against an injectable programme, where a minimum of roughly five sessions at weekly intervals is the standard shape and the effect arrives late in the course. If you have four days in Seoul, the difference is not academic.

Booking sequence is simple. You describe the chin in a message, a director tells you plainly whether photographs suggest fat or laxity, and a same-day slot is usually possible once you are in the building for the consultation. If the assessment in the room contradicts what the photographs suggested, the appointment stops there and no procedure is performed.

Suitability

The straightforward candidate has visible submental fullness with skin that still has reasonable tone: pinch the area, let go, and the skin springs back within a moment. That person gets a predictable jawline from a single attendance, because there is something to remove and a surface capable of settling back down over it once the volume is gone.

Several presentations are declined outright, and it is fairer to publish the list than to let somebody book a flight around it. Severe skin laxity is the most common refusal: emptying a pocket under slack skin can leave the area looking worse than it started, so those patients are redirected to lifting options and are told plainly that extraction is the wrong department for them. Pregnancy and breastfeeding pause the conversation entirely. An active infection anywhere on the chin, jaw or neck postpones the appointment until it has resolved. An uncontrolled systemic condition, or medication that has not been disclosed on the intake form, sends the case back to consultation before anything is booked.

Two further requests get turned down regularly. The first is the patient whose profile is set by jaw position and bone, with very little fat available to remove, where the assessment says so on the day and the plan stops there. The second is anyone treating this as weight management. Extraction under the chin addresses a defined local pocket and has no bearing on body weight, and a director who suspects that is the real goal will say the procedure is not indicated.

What Changes

How the jaw shadow reads as swelling drains away.

Volume drops during the appointment, because the fat is physically gone by the time the support garment goes on. What you see immediately afterwards is not the finished contour: the area swells, and swelling in the submental space is deceptive because it sits exactly where the fat used to be. Patients regularly report that the chin looks fuller on day two than it did before they arrived. That is expected and it passes.

The line becomes readable as the swelling settles over the following days and weeks, and the neck-to-chin angle is usually what people notice first, ahead of any change they can measure. Skin tone at the outset sets the ceiling on the result, which is the reason so much of the consultation is spent on the pinch test and comparatively little on the sheer amount of fat present. Individual results vary between two people who receive identical treatment, and no photograph of somebody else predicts what your jaw will do.

Named Risks

Risks a director names out loud before a chin is treated.

Bruising and swelling are routine and expected under the jaw, and both are more visible here than on a covered part of the body simply because the area is on show. Numbness of the chin and the skin along the jaw is common in the early period as sensory nerves recover from being handled; it typically resolves, and it is described at consultation on a scale of weeks.

Contour irregularity is the risk that most deserves attention, because the submental surface is unforgiving. Removing unevenly, or removing from a compartment whose skin cannot re-drape, can leave a dip, a ridge, or an asymmetry between the two sides that is visible in ordinary light. Nerve irritation is the second named concern: the marginal mandibular branch of the facial nerve runs close to the working field, and irritation of it can temporarily pull the smile out of balance on one side. Infection at the entry point, prolonged tenderness, and a firm area of scar tissue under the skin all belong on the same list. None of these is described here as impossible, and a consent conversation that leaves them out is not a consent conversation. Aftercare

The first week with a chin support garment.

You leave wearing a support garment and you are told for how long to keep it on, including overnight. Sleeping propped up for the first few nights makes a noticeable difference to morning swelling. Soft food for a day or two spares the jaw unnecessary work, and heat, alcohol and hard exercise are set aside during the early phase because all three make swelling worse in an area where swelling is already the main complaint.

Desk work is realistic the same day. Being photographed is a different question, and anyone with an event in the calendar should count in weeks. Flying is generally comfortable once the initial phase has passed; patients on short trips are asked to leave a margin between the appointment and the departure gate so that a director can see the area once before you go. If something looks wrong after you land, you write to the clinic with photographs and a director answers, and that channel stays open regardless of how long ago the visit was.

Scientific Evidence

Published papers behind awake fat removal of the neck.

Three references are listed below so a reader can go to the sources directly. The first examines tumescent liposuction and the efficacy of a lower lidocaine dose at 400 mg per litre (Böni, 2010). The second reports on the safety of liposuction using exclusively tumescent local anesthesia across 3,240 consecutive cases (Habbema, 2009). The third describes liposuction of the neck and jowls by a five-incision method combining machine-assisted and syringe aspiration (Langdon, 2000). Those sentences state what each paper is titled and nothing beyond it.

None of these papers studied G Clinic, none describes the V-Tight protocol used at this address, and none of them supports a prediction about what your own jawline would do. Published literature of this kind establishes that a method category has been examined in the peer-reviewed record. It does not transfer to an individual consultation room, and it is cited here as background only.

How long am I in the building on the day?

Plan for about an hour from arrival to leaving, assuming the consultation on the day confirms you are suitable. The ultrasound and extraction portion itself takes roughly twenty minutes; the rest is assessment, marking, numbing, and fitting the support garment before you go. There is no admission, no fasting, and no need to arrange somebody to collect you afterwards.

Will I be asleep during the procedure?

No. The area is numbed with local anaesthesia and you stay awake, sitting up between checks and able to talk with the director the whole time. Sedation is available as an option for patients who prefer it and carries an additional cost. The specific agent is discussed with you in the consultation room and is never named in published material, which is a rule of Korean medical advertising.

Does the fat under my chin grow back afterwards?

The fat cells removed during the appointment are gone from that pocket. What the procedure cannot do is freeze your body in place: significant weight gain afterwards will still be stored somewhere, including in remaining cells around the treated area, and a jawline can lose definition again for that reason. My double chin looks like loose skin. Would you still treat it?

Probably not with extraction. Where the fullness comes from skin that has lost its recoil, emptying the pocket underneath can leave the area looking looser than it did at the start. What risks would you name to me before I sign anything?
